Appointments

Walk-ins are always welcome but you may have to wait for a representative or will be asked to make an appointment and come back.

Your best option is to request an appointment prior to any visits using the online form below (please do not send personal information such as your Social Security Number).  We will review your request, contact you for an appointment, and advise you on types of documentation to bring to your appointment. We can also point you to the correct office if your issue isn't something handled by Veterans Services. 

After you send the appointment request, complete the Intake Form and plan on bringing these documents with you (if applicable):

Veteran

  • DD214 or other discharge certificate
  • Income verification (pay stubs, award letters, or other documents showing monthly income)
  • Bring in dependent information for each marriage, as applicable (e.g. marriage certificates, social security numbers, spouse income information, death certificates, divorce papers, etc.)
  • Birth certificate or adoption record for dependents under 18 or 23 and attending school
  • Copies of your military and civilian medical records
  • Photo ID

To Establish Relationship as a Veteran's Dependent

  • DD214 or other discharge certificate 
  • Marriage certificate
  • Birth certificate or adoption record for dependents under 18 or 23 and attending school
  • Death certificate
  • Information on previous marriages for veteran and current spouse, as applicable (e.g. marriage certificates, social security numbers, current spouse income information, death certificates, divorce papers, etc.)
  • Photo ID

We can assist with requesting copies of some federal and state records.
